Environmental Services is responsible for reviewing all well permit applications for referral to the Arizona Department of Water Resources on parcels 5 acres and smaller.

Well data in Arizona is now available online through Arizona Department of Water Resources.

NOTE: Some of the information needed to complete this form can only be obtained from an Arizona Department of Water Resources licensed well driller.
