Yavapai County Community Health Services Launches Community Outreach Survey

YAVAPAI COUNTY, Arizona. – Yavapai County Community Health Services (YCCHS) is launching a comprehensive community outreach survey designed to gauge resident’s awareness, utilization, and perceptions of their public health resources, including Community Health Center of Yavapai (CHCY) services and various YCCHS programs.
The objective of the survey is to gather essential, real-world data that will directly inform YCCHS’s strategic planning and improve how services and communication is delivered across the county. The feedback received will help identify gaps in service, highlight successful programs, and ultimately ensure that YCCHS/CHCY resources are helping the maximum number of people as possible.
Yavapai County residents are strongly encouraged to participate. The survey is now open through January 7th and is available in both English and Spanish. All responses are anonymous and will be used solely for the purpose of community health improvement and resource allocation. The assessment should take approximately 10 minutes to complete.
The survey can be completed online by visiting the links down below or by scanning the QR Code with a smartphone camera. For residents who prefer a paper format, physical copies are also available at all Yavapai County Library Branches.
