The CHCY healthcare medical and behavioral health team are a collaborative assembly of dedicated, caring and compassionate professionals who seek to provide the best care possible to you and your family. The CHCY staff work together to assure patients are provided with the services and assistance to achieve greater overall health and an improved quality of life.

Dental Services

CHCY offers convenient and affordable Dental Services to meet your family’s dental needs. As a Federally Qualified Health Center, all dental services are available on a sliding fee scale to eligible patients. We welcome AHCCCS, and private insurance.

Family Planning/Reproductive Health

Services are available to women and men who seek confidential family planning and related services. Services include FDA-approved birth control methods, screening and treatment of STIs, pregnancy testing and options counseling, Pap smears and HIV testing. Patients under 100% of the Federal Poverty level are asked for a donation; those above 100% are charged according to a sliding fee schedule.

Pediatrics

Our compassionate, knowledgeable and supportive team of pediatricians provide high quality pediatric specialty care for children from infancy through the age of 18.

Behavioral Health

Adult, outpatient, behavioral health counseling and psychiatric services are available to Community Health Center patients who are receiving their primary care medical services from the Community Health Center of Yavapai and are not eligible for behavioral health services elsewhere. These counseling services are provided to children and adolescents on a limited basis.

Well Woman

Community Health Center of Yavapai is proud to be a part of National Breast and Cervical Cancer Early Detection Program, (NBCCED) known in Arizona as the Well Woman HealthCheck Program.

 Community Health Center of Yavapai (CHCY) is dedicated to providing:

  • Behavioral Health Including Counseling and Psychiatry
  • Reproductive Health
  • Pediatrics
  • Pharmacy Assistance Programs
  • Well-Woman HealthCheck
  • Certified Application Counselors
  • Emergency Food Boxes

CHCY offers a sliding fee scale program for uninsured and underinsured patients. Patients are not denied care for their inability to pay for services.

CHCY provides evidence-based medicine, same-day and tele-health appointments.

Established patients may also access the after hours on-call service.
