Primary Care

CHCY primary care providers work collaboratively with the health care team to offer patient centered services including disease prevention, health maintenance, patient education, and diagnosis/ treatment of acute and chronic illnesses. Same day appointments are available at each office as well as an afterhours on call service for established patients.

CHCY offers affordable dental services to Yavapai County residents.  Accepting AHCCCS and private insurance, CHCY also provides dental care on a sliding fee scale to income eligible patients.

Family Planning/Reproductive Health

Services are available to women and men who seek confidential family planning and related services. Services include FDA-approved birth control methods, screening and treatment of STIs, pregnancy testing and options counseling, Pap smears and HIV testing. Patients under 100% of the Federal Poverty level are asked for a donation; those above 100% are charged according to a sliding fee schedule.

Our compassionate, knowledgeable and supportive team of pediatricians provide high quality pediatric specialty care for children from infancy through the age of 18.

Behavioral Health

CHCY offers in person and telehealth appointments for patients seeking behavioral health services. Our counselors and psychiatric providers see patients starting at age 6 and through adulthood.

Well Woman Health Check

CHCY is proud to offer breast and cervical cancer screenings via the Well Woman Health Check Program.  Income eligible women are able to receive care coordination services to assist them with cancer screening and treatment options if needed.

CHCY offers a sliding fee scale program for uninsured and underinsured patients. Patients are not denied care for their inability to pay for services.

CHCY provides evidence-based medicine, same-day and tele-health appointments.

Established patients may also access the after hours on-call service.
